UW-Eau Claire and our Services for Students with Disabilities (SSD) staff are committed to providing an equal educational opportunity for all students. Students with disabilities are offered a variety of services and accommodations to insure that both facilities and programs are accessible.
The SSD website is designed to provide an overview of the resources available and the policies and procedures in place to facilitate accessibility and academic success. We encourage you to contact us with any questions or comments.
The Services for Students with Disabilities (SSD) office facilitates accommodations and services to students with disabilities that ensure equal access to university programs, services and activities. UW-Eau Claire is committed to providing all students with accessible educational opportunities. UW-Eau Claire follows UW System Policy, Section 504 of the Rehabilitation Act of 1973, and Title II of the Americans with Disabilities Act of 1990 (ADA).
Disability: A physical or mental impairment that substantially limits one or more of the person's major life activities; a history of such an impairment; or being regarded as having such an impairment. UW-Eau Claire and UW-Eau Claire – Barron County follow UW System Policy, Section 504 of the Rehabilitation Act of 1973, and Title II of the Americans with Disabilities Act of 1990 (ADA).

Our office reviews all requests for disability-related accommodations for students.
To receive accommodations, please start by filling out a short application. This is so that we can gather basic information and get to know students a little more.
Please note: If you are working to get documentation, we still encourage you to submit the application for accommodations. Documentation can be uploaded or provided after submitting the application. However, the application does include a place to upload documentation if it is in an electronic format.
Disability Documentation Requirements
To ensure the provision of reasonable and appropriate accommodations on the basis of a disability, students requesting accommodations must provide documentation of their disability as defined by federal law. Title II of the Americans with Disabilities Act (ADA) of 1990 as amended and Section 504 of the Rehabilitation Act of 1973 define a disability as a physical or mental impairment that substantially limits one or more major life activities.

Documentation is not required at the time of submitting your online application for accommodations. It can be uploaded or provided after submitting an application. However, the application does include a place to upload documentation if it is in an electronic format. The form below can be used to help facilitate gathering the necessary documentation or if you have other forms of documentation that follow the guidelines above, that can be submitted as well. 504 Plans and IEPs from High School can be submitted.
In addition to uploading documentation to the online application, it can be emailed to us at ssd@uwec.edu, faxed to 715-318-5559, or dropped off in our main office.

Once an application and documentation has been submitted, our Accommodations Coordinators will review. Students will then be contacted by email (UWEC email) to arrange an intake appointment or to request additional documentation if that is needed. Accommodations will then be discussed at an Intake Meeting. We offer these virtually or in person.
